Near Miss Reporting

Aboard ship, all personnel have the right and the responsibility to report defects in the vessel’s equipment and procedures, and all near misses. Near misses are events that, under slightly different conditions, could have released the potential of a hazard to become active and cause harm. Each near miss embodies a lesson learned that could help others avoid similar circumstances if the knowledge is circulated throughout the ranks of vessel and company personnel and retained over time. The process should be formalized into a comprehensive near miss reporting system, rather than left to chance.
